New Clients: You will receive an emailed invitation from My Sevocity Heathlink to join the patient portal. You will be prompted to set a username and password. Once you're logged into the portal, in the Past History tab, you will find a questionnaire to fill out prior to your first scheduled appointment. The portal saves your progress on the form, so you are welcome to take your time.
